Job Description
Key responsibilities.
- Be the first point-of-contact for people contacting our service. Managing the administration of intake referrals and the triage assessment of these referrals.
- Using our triage assessment framework to establish an understanding of clients' needs, risks and urgencies, adopting a client-centred and trauma-informed approach to supporting the triage of these people into the appropriate service and program.
- Maintaining high standards of documentation and record keeping, ensuring client information is recorded in our client information management system.
- Developing and maintaining rapport with clients and strong relationships with referrers, to support optimal continuity of care.
- Being an ambassador for change within the organisation, approaching transitions with flexibility and collaboration.
About you:
Essential Criteria
- Bachelor or postgraduate qualifications and current registration in either social work, psychology, occupational therapy or nursing, with accreditation or formal training in mental health.
- Skills and competence at completing bio-psycho-social intake assessments and conducting crisis risk assessments.
- Ability to work autonomously to manage daily workload in line with service delivery expectations and an ability to work collaboratively as part of a multidisciplinary team to assist clients in accessing the support they require.
- Compassionate approach to clients experiencing physical and mental health challenges and emotional distress.
- High level of interpersonal and communication skills including excellent phone manner.
- Strong organisational skills, specifically related to client information management, referrals, intake and triage decisions.
- Culturally sensitive practice when working with Jewish clients and a capacity to connect with people across a range of client groups
- Ability and willingness to work with existing client information management systems, and maintain high standards of client documentation and reporting
- Computer literacy skills
- Up to date WWCC, NDISWC and Criminal Record Check Clearance
- Experience working in a triage and/or intake role, across mental health, aged-care, disability or child and family services.
- Experience working in mental health, aged-care, NDIS or social welfare services.
- Knowledge of trauma-informed care and legislation relevant to client presentations.
- Experience working with those affected by family violence.
- Experience working through organisational change.
