Reporting to the Home Care Manager, your primary focus will be to provide effective and efficient day-to-day case management services. You will ensure the delivery of flexible, individually tailored packages of care and support to a range of community clients requiring DVA Nursing services & Home Care and Private Nursing services.
Your responsibilities will include:
- Providing person centred case management in partnership with consumers and RNs in line with DVA Community Nursing Services guidelines, Aged Care Quality Standards and Home Care Packages Program
- Working collaboratively with Client Case Officers, facilitating consumer referrals to supports and services in relation to their unique wishes, goals, and needs.
- Assisting in the development of plans and referrals to Allied Health Specialists with consumers identified needs and relevant health information.
- Facilitating rehabilitation appliances and continence product ordering for DVA Community Nursing Consumers.
- Daily interface with Client Case Officer to provide guidance, clinical directives, and delegation of consented care for consumers of concern and those with complex and high care consumer needs.
- Identifying and communicating to all relevant staff, mobile workforce, and interdisciplinary team, any priority clinical health issues and information affecting consumers, to ensure safe and effective delivery of consumers personal and clinical care needs.
- Maintaining and overseeing administrative tasks including the coordination of Medical Officers correspondence, maintaining clinical stock items for services, and updating Home Care Packaged clients clinical support plans and maintaining clinical and incident management data.
- As a Registered Nurse, attend to delivering client clinical services in the home, as required by the service.
- Current Nurse Registration with APHRA
- Experience in the provision and coordination of care services to elderly members of the community.
- Experience in organising diverse workload and coping with change
- Strong customer service / relations skills and a commitment to providing quality services and implementing continuous improvement.
- Sound knowledge of the DVA Services, Home Care Packages, and Aged Care Quality Standards.
- Ability to manage multi tasks concurrently, be flexible in dealing with changing priorities and have the ability to manage conflicting demands.
- Highly developed interpersonal and communication skills including an ability to deal with conflict constructively.
- Ability to be self-directed and self-motivated with strong forward planning and organisation skills.
- Drivers Licence Class C.
