About the Role

You will provide clinical leadership and support the multidisciplinary team to achieve collaborative care utilising team engagement, advanced clinical skills, knowledge and problem solving to effectively coordinate care delivery and manage complex situations. Your ability to build connective partnerships with patients to perform comprehensive assessments, develop and implement care plans and evaluate care outcomes will be essential to your success in the role. You will also have a commitment to innovated improvements and provide support and direction to less experienced team members.
