Position Profile: Provide education and support to Chronic Kidney Disease (CKD) clients and their families. Support the CKD client and their family to prevent progression towards End Stage Kidney Disease. Improving health outcomes through education and support.
To be successful in this role, you will have:
- Experience in Chronic Kidney Disease process management and education.
- Well-developed interpersonal skills and ability to communicate across a diverse spectrum of the population.
- Understanding and respect for the needs of Aboriginal culture in providing effective health service delivery to Aboriginal people.
