About the opportunity:Join our
Community Aged Care team as a
Clinical Nurse! Lead our multidisciplinary team, ensuring safe and effective care for our
Community Aged Care consumers. Collaborate with the Clinical Services Manager and other key staff to maintain accreditation, drive compliance, foster continuous improvement, and address training needs. An exciting opportunity awaits!
Key Responsibilities- Coach and mentor staff on clinical matters through positive leadership.
- Assess nursing staff competencies and maintain effective performance development.
- Collaborate with managers to identify staff training needs and schedule delivery.
- Lead and deliver clinical nursing care to consumers.
- Coordinate nursing assessments in consultation with consumers and the team.
- Engage with coordinators and managers on admissions and discharges.
- Provide clinical input in the development of client health assessments.
- Make referrals to other healthcare professionals as needed.
- Lead support program auditing and accreditation activities.
Skills & Experience:- Manage clinical duties effectively as a Clinical Nurse.
- Provide direct clinical care, support, guidance, and education in community settings.
- Work autonomously and collaboratively within teams.
- Demonstrate high-level evidence-based clinical and problem-solving skills.
- Understand and apply Aged Care Accreditation standards and guidelines.
- Familiarity with Home Care Packages (HCP) and related programs.
- Demonstrate awareness of Clinical Governance in Community Aged Care.
- Demonstrate outstanding time management and organisational skills.
Essential requirements:- Bachelor of Nursing and Registration with AHPRA
- Post Graduate qualification in relevant field of speciality (Community and Primary Health Care Nursing or similar)
- Current First Aid Certificate
- Current National Police Clearance – Aged Care
- Current DHS NDIS Worker Screening Check
- Current SA Drivers Licence
- Reliable vehicle and comprehensive vehicle insurance
