This position will work in close liaison with staff from Primary Care and Community Heath, acute facilities, General Practitioners, Specialists, other healthcare providers, Non-Government agencies and consumers and their carers and families to improve outcomes for people with a Complex Aged Care condition. The position will work collaboratively with other specialist Aged Care support nursing positions (including Nurse Practitioners, CNCs and CNS2) to ensure a whole of district approach to the provision of community care to the target client group.An eligibility list may be created for future vacancies.About us:

Booster doses are highly recommended for all health care workers who have completed the primary course of COVID-19 vaccinations.Employment of a temporary visa holder may only occur if no suitable permanent resident or citizen of Australia has been identified and must not exceed the duration or conditions associated with the current visa.Selection Criteria to be Addressed: * Current Registered Nurse Division 1 with the Australian Health regulation Practitioner Regulation Agency (AHPRA) with five years full time equivalent post qualification experience, and a minimum of three years full time equivalent experience in Aged Care nursing and post graduate qualifications (relevant to the specialty area of practice)
- Proven clinical leadership skills and an ability to work collaboratively within a multidisciplinary framework with a commitment to excellence in evidence based nursing practice
- Demonstrated effective communication and inter-personal skills across a variety of mediums and an understanding of the practice development framework.
- Proven ability to implement changes and develop direction in clinical practice by the utilisation of evidence based research.
- Demonstrated skills in strategic and clinical service planning including an understanding of organisational behaviour, change management and cultural change processes.
- Demonstrated skills in clinical teaching, competency in the use of contemporary information systems and experience in the development of specialty education programs.
- Demonstrated clinical expertise in client-centred consultancy with the ability to work independently.
- Current valid NSW Drivers Licence (including P1 or above) and willingness to use for work purposes. This may include driving long distances across the LHD.
