The successful applicant will provide leadership in the ongoing development of clinical practices in relation to patients with complex health needs associated with heart failure. The successful applicant with provide clinical advice to patients, carers, and other health care professionals whilst monitoring the clinical care of patients transitioning from the acute settings to home by providing clinical support to improve patient care and outcomes.
