With a booming, culturally rich and diverse community, and less than an hour's drive from the CBD, Melbourne's North is fast becoming the place to be!The Role:An exciting opportunity exists for a experienced nurse to join the Hepatology & Gastroenterology team at Northern Health.This role requires a Hepatology Gastroenterology Clinical Nurse Consultant (CNC) to work as part of a team in our Outpatient & Liver Well at Home program.Fixed Term- 12 months (Parental Leave relief)40 Hours/Fortnight. 0.5 EFT.Commencing early July 2024.The Clinical Nurse Consultant (CNC) provides direct clinical care to patients with liver disease. The Northern Health Hepatology Service is one of the fastest growing Hepatology referral services in Australia. The CNC will work collaboratively with the multidisciplinary team to provide evidence-based care to this patient group.The Hepatology CNC plays a pivotal role in the co-ordination and management of patients with liver disease in the ambulatory environment, and be intricately involved in the delivery of a Liver Well at Home initiative at Northern Health. The role requires a detailed understanding of liver disease and treatments and will be responsible for providing patient education and support to optimise self-management, monitoring of medication adherence and treatment impacts/outcomes. The CNC will be required to undertake home visits to patients with liver disease, perform assessments, and institute changes of therapy in consultation with the Head of Service, Hepatology. The CNC will maintain relevant databases and contribute to research as required.To be successful in this role you will have:
- AHRPA registration as Registered Nurse
- Post graduate qualifications or working towards
- Working knowledge of hepatology and gastroenterology nursing
- Excellent organisational skills with a flexible approach to changing situations
- Ability to be part of an effective team, as well as work autonomously
- Solid verbal and written communication skills with the ability to adapt language to the requirement of patients of diverse populations, pharmaceutical companies and senior consultants
- Competencies in Microsoft word, excel, outlook and a familiarity with data bases
