Job description

Dementia Outreach Service are looking for a Clinical Nurse Consultant to join their team. The Dementia Outreach Service is a multidisciplinary team that provides a service across Residential Aged Care Facilities and the community within the Metro South Health catchment. The Clinical Nurse Consultant (CNC) provides direct and indirect care as required and works in collaboration with the Nurse Practitioner, Nurse Educator and wider Metro South health care team support the clinical unit in provision of safe quality healthcare through exemplary clinical nursing standards of practice for the specialty of cognitive impairment/dementia.

About the Role

The purpose of this role is to provide clinical expertise and consultancy through advanced clinical leadership relevant to the speciality of cognitive impairment/dementia.

  • Advanced level of autonomous clinical and professional practice.
  • Advanced level of decision making in collaboration with a multi-disciplinary team.
  • Undertake operational leadership.
  • Work collectively to apply the principals of clinical governance.
  • Apply professional and clinical expertise in collaboration with nursing and multi-disciplinary stakeholders.
  • Responsible for enacting strategies that support a work-based culture that promotes and supports education, learning, research and workforce development.
