Requisition ID: REQ480823To be considered for this position you must be an employee of NSW Health and its Affiliated Health Organisations.The Integrated CareClinical Nurse Consultant (CNC) promotes an integrated person-centred care model which advocates for effective and efficient care focusing on the unique physical, mental and social needs of the individual and fosters a collaborative partnership between patients/carers, service providers and their General Practitioner/primary care to optimise health outcomes and develop appropriate care plans.The Integrated Care CNC plays a major clinical role through the provision of expert clinical assessment, referral and coordination of health and community services for people with severe chronic disease and complex care requirements living in the community who are at risk of deteriorating health that may result in declining quality of life and an acute hospital admission.The Integrated Care CNC provides expert clinical advice to patients, carers and other health care professionals within SESLHD, community organisations, primary care/General Practice, Non-Government Organisations (NGOs) and other government departments to support integration of care.The Integrated Care CNC utilises a flexible and adaptive approach to patient care within a changing health system and supports/coaches patients to coordinate and navigate across the health care system and to connect with other community and hospital-based coordination services as required in drug and alcohol, mental health and primary care.Employment of a temporary visa holder may only occur if no suitable permanent resident or citizen of Australia has been identified for this position following suitable labour market testing.All workers and new recruits are required to receive 2 doses of a Therapeutic Goods Administration approved or recognised COVID-19 vaccine to commence employment/ engagement or continue to work within a NSW Health service.A worker and new recruit will be considered compliant if they have a medical contraindication to all available Therapeutic Goods Administration approved or recognised COVID-19 vaccines and provide medical contraindication evidence in line with the policy requirements.In addition, all Category A workers and new recruits are required to receive one dose of the seasonal influenza vaccine annually to be considered compliant.Category A workers and new recruits who are non-compliant with seasonal influenza vaccination or have a medical contraindication to influenza or COVID-19 vaccinations must comply with all other infection control risk reduction strategies as directed while working in a Category A position.SESLHD is committed to creating a workplace that reflects the diversity of our community. - Current Authority to Practice as a Registered Nurse, Nursing and Midwifery Board of Australia.
- Minimum five years full time equivalent post graduate qualification experience and a post graduate qualification in a relevant field of practice.
- Demonstrated experience working with and extensive knowledge of the broader health and social issues that people with mental health and/or drug and alcohol concerns.
- Demonstrated ability in communicating effectively with multiple care providers including general practitioners, other primary care providers, and members of multidisciplinary teams as required.
- Demonstrated effective computer skills in using electronic medical records and the Microsoft suite of products(Outlook, Word, Excel, PowerPoint).
- Demonstrated clinical expertise and the ability to apply clinical expertise in person-centred care planning.
- Demonstrated ability to work independently and within a multidisciplinary team.
- Current unrestricted Class C NSW Driver's Licence.
