Your opportunity
The Clinical Nurse Consultant Nurse Navigator is accountable at an advanced practice level for the provision of specialised knowledge and advanced clinical practices to a cohort of patients within the rural/ remote context.
The Nurse Navigator role will utilise advanced clinical skills and professional decision making to deliver a safe and efficient patient journey across the care continuum through the reduction of service fragmentation, educating and empowering individuals and coordinating patient care in a positive way.
