About the Role
The Community Services Team is a multi-disciplinary team that provide acute and post-acute care via hospital avoidance (MAPS) and hospital in the home (GEMRHITH) interventions. Care is provided across seven days, in various settings including hospital clinics, community centres and consumers places of residence.
The Clinical Nurse (CN) role provides advanced clinical and problem-solving skills, expert planning and coordination skills in the clinical management of Community Services Team patients with complex care needs, ensuring quality standards are met. This role works autonomously within the healthcare team structure and is pivotal in the planning, managing and delivering of community specialist nursing care. The CN provides clinical leadership for professional and clinical practice, education and research. The role supports in management activities including portfolio responsibilities. The role will provide care to community clients in either the home or clinic setting to deliver a wide range of Acute (HITH, GEMRHITH patients) and Post-Acute (MAPS - Multidisciplinary Avoidance and Post-Acute Service patients)
