Full-time

Rosewood Care Group (Inc.) is searching for an experienced, dynamic, and passionate nurse to lead the clinical services team at our brand new, state of the art 152 bed facility in West Perth.

This position will report into the Operations Manager.

Key Responsibilities:

  • Upholds the Mission, Vision, and Values of Rosewood in the workplace.
  • Provide a high level of resident-focused care.
  • Provides leadership and supervision of all Care Staff (Registered Nurses, Enrolled Nurses, Carers) in provision of practices aimed at achieving optimum resident outcomes.
  • Works with all Care Staff to ensure that documentation and assessments provide sufficient information to enable the development of individualised care plans that reflect residents’ care needs.
  • Coordinates resident-focused care delivered by Care Staff.
  • Evaluates residents’ response to interventions and then reviews and revises care plans in collaboration with all stakeholders.
  • Liaises with other health professionals, such as doctors and allied health professionals, to ensure the provision of high-level care (in accordance with Privacy and other Policies and Procedures).
  • Ensures clinical assessments and individualised care plans are developed and reviewed regularly and reflect residents’ current care needs.
  • Assists in the collation of AN - ACC submissions in conjunction with the AN - ACC Coordinator
  • Suggests changes to clinical procedures, processes, and policies to ensure agreed standards and outcomes are met.
  • Identifies opportunities for innovations in nursing practice.
  • Identifies own and other staff education and development needs to ensure a skilled and competent workforce that meets the ongoing needs of residents. Assist the Operations Manager with staff performance reviews as required.
  • Participates, as required in processes such as clinical audits and reviews to ensure compliance with the Accreditation Standards.
  • Ensures medication and personal care competencies of all Care Staff, following their employment and then regularly as per relevant policy.
  • Communicates effectively with a variety of people and contributes to teamwork within the workplace.
  • Ensures appropriate use of resources.
  • Maintains confidential information about clients, staff, and the organisation.
  • Works with others to assist in the resolution of problems/issues.
  • Contributes to the continuous improvement of the organisation.
  • Attends staff, quality and other meetings as required.
  • Adheres to a clinical budget.
  • Contributes to the provision of a safe working environment within the organisation.
  • Ensures work practices demonstrate compliance with requisite Regulations and Legislation relevant to position.
  • Provides recommendations to the Operation Manager in relation to clinical improvements which will result in positive outcomes for the organisation.
  • Ongoing high-level review of clinical care with trending, analysis, and actions to ensure safe and quality care are delivered.
  • Ensure all NACMQIP, Clinical Indicators, Moving on Audits (MOA) are completed on time, with data analysed, with recommendations and presented to Clinical Governance Committee and linked to Rosewood’s Continuous Improvement Plan.
  • Critically analysis data, implement corrective actions on identified gaps, trends, and further opportunities for Continuous Improvement.
  • Assist the Operations Manager with any care-related complaints ensuring there is open disclosure, clear communication with all parties and opportunities for improvement and education are reviewed and implemented.

Essential Criteria:

  • A minimum of five years’ experience at Clinical Nurse level.
  • Experience in aged care, extended care, or rehabilitation services.
  • Experience in providing leadership in the provision of care services.
  • Ability to work as a positive and productive team member.
  • Effective written, oral, and interpersonal communication skills.
  • Well-developed analytical and problem-solving skills.

Qualifications:

Prerequisite:

  • Relevant tertiary qualifications. Registered by the Australian Health Practitioner Regulation Agency as a Registered Nurses.

Desirable:

  • Solid understanding of the Australian National Aged Care Classification funding model and aged care standards and guidelines.
