About you:
The Clinical Nurse Specialist Grade 2 provides evidenced-based nursing care to people of all ages and cultural groups, including individuals, families and communities. The Clinical Nurse Specialist Grade 2 assesses, plans, implements and evaluates nursing care in collaboration with individuals and the Multidisciplinary health care team so as to achieve goals and health outcomes. The Clinical Nurse Specialist Grade 2 takes an extended leadership role in the coordination of nursing and health care within and across different care contexts to facilitate optimal health outcomes. This includes appropriate referral to, and consultation with, other relevant health professionals, service providers, and community and support services. The Clinical Nurse Specialist Grade 2 practices independently and interdependently with an extended scope of practice, autonomy and decision making, assuming accountability and responsibility for their own actions and delegation of care to registered nurses, enrolled nurses and health care workers. Delegation takes into consideration the education and training of nurses and health care workers and the context of care.

Position Duties:

  • Provides individual assessment for the patient’s physical, mental, psychosocial and spiritual needs.

  • Provides short to medium term individual case management with clearly defined goals that utilise expertise in the mental health management of older people with mental health problems and mental illness

  • Co-ordinate Mental Health Review Tribunals and Community Treatment Order hearings and provide written reports as required

  • Case Manage clients who are managed on Community Treatment Orders

  • Provides a crisis assessment and intervention service for Older Person’s mental health patients of the St Vincent’s Mental Health Service and their carers

  • Participation in the provision of education and training within the Inner City Health stream, including the mentoring of students on placements.

  • Co-ordinates care with the other members of the health service.

  • Organizes work demonstrating flexibility and appropriate prioritizing

  • Establishes and promotes appropriate referral and feedback systems.

  • Practices at a consistent expert level and provides leadership within the team.

  • Involvement with consumers through various mechanisms such as attendance at Consumer Consultative Committee, inpatient, and consultation, meetings and working parties.

  • Promotion and commitment to the implementation of quality improvement activities related to the care and treatment of Older Person’s mental health consumers and their carers

Selection Criteria:
1. Personal integrity and demonstrated commitment to the Philosophy, Mission and Values of Mary Aikenhead Ministries and St Vincent’s Health Australia
2. Currently registered as a Registered Nurse (Division 1) with the Australian Health Professionals Regulation Agency

3. Relevant post-registration qualifications and at least three years’ experience working in mental health and/or older person’s care

4. Demonstrated ability to work within the interdisciplinary team at a senior level

5. Demonstrated commitment to ongoing professional development and demonstrated effective communication and interpersonal skills

6. Demonstrated effective computer literacy with email and an electronic patient record system

7. Demonstrated evidence of integration of theoretical knowledge and clinical skills, critical thinking and analysis in their daily practice as a registered nurse

8. Demonstrated effective time management skills and ability to prioritise competing workloads with in a complex environment
