The role of Clinical (RN) Case Manager offers a great deal of variety of tasks, enabling you to utilise ALL of your skills!
As a Clinical (RN) Case Manager you will provide person centred care to your allocated case load in the community, ensuring a safe environment where you will apply your clinical skills to improving their quality of life. This position will be involved in undertaking initial health assessments and onboarding of new clients.
The role will have you undertaking assessments such as a comprehensive health assessments, continence and bowel assessments and wound assessments to name a few. You will have excellent attention to detail which is crucial to the role along with your superior report writing/assessment skills. Experience writing and tailoring reports for NDIS participants will be highly regarded. You will design and review care plans for clients with Complex Health Care needs.
You will be involved in the care of our complex clients, providing clinical oversight along with mentoring, developing and supporting our community team of Nurses and Support Workers, ensuring we are providing the best client centred care that we can. Scheduling oversight will also be part of the role.
You will have excellent clinical judgement and decision making skills and will utilise your clinical skills undertaking nursing services from time to time such as catheter changes, wound management and diabetic management.
Some travel will be involved as we service an area from Northern New South Wales to the Sunshine Coast. From time to time, we may also undertake assessments and training which may involve intrastate travel with a night or two away from home. On the ad hoc occasion when overnight stays are required this will be negotiated at a time that is mutually convenient!
To be successful in the role you will have:
- Qualified Registered Nurse with a minimum of 2-5 years community experience
- Ability to work collaboratively in our multi disciplinary team
- Excellent community and clinical problem solving skills
- Highly developed verbal and written communication skills
- Confidence in managing your time effectively, including an ability to work autonomously and part of a team
- Well developed interpersonal skills that you use to establish excellent working relationships with both internal and external stakeholders
- Comfortable using technology (mobile devices) for accessing rosters and recording customer assessments and documenting feedback
- Blue card and NDIS Workers Screening Clearance or willingness to obtain
- Full, unrestricted AHPRA registration and legal working rights in Australia
- First Aid and CPR
- Current Australian Driver’s Licence and, as you will use your own vehicle, proof of your comprehensive car insurance.
- Be available to participate in the On Call roster, once every 8-10 weeks
