Role Overview:

Key Responsibilities
- Coach and mentor staff on clinical matters through positive role modelling.
- Assess staff competencies and ensure effective performance development and reviews.
- Engage with Coordinators, Team Leader Coordination, and Registered Nurses on admissions and discharge summaries.
- Provide clinical input in developing health assessments for HCP and TCP clients.
- Case manage complex clients with high clinical needs in collaboration with RNs, Coordinators, and Team Leader.
- Audit and manage medication administration following UnitingSA policies.
- Provide clinical guidance and professional support on client and care-related issues.
- Led a team in a recent leadership position
- Managed clinical and practice operations for 3+ years as a Clinical or Practice Manager
- Provided direct clinical care, support, guidance, and education in a community setting
- Communicated clearly, adapting style for consumers and staff needs
- Engaged in sensitive, open, and effective communication with stakeholders
- Demonstrated understanding of Aged Care Accreditation process and Quality Standards
- Proficient in Home Care Packages (HCP) and Transitional Care Program (TCP)
- Extensive knowledge of Aged Care Quality Standards and Clinical Governance in community aged care
- Built and maintained professional relationships with clients and service providers
- Exhibited excellent customer service, time management, and organisational skills
- Bachelor of Nursing or Master of Physiotherapy
- Registration with the Australian Health Practitioner Regulation Agency (AHPRA)
- Evidence of ongoing clinical professional development
- Current First Aid Certificate
- Current SA Drivers Licence
- Reliable vehicle and comprehensive vehicle insurance
- Current DHS NDIS Worker Screening Check
- Current National Police Clearance
- Current Covid-19 Vaccinations (2 doses + 1 booster)
