So, what will be your part in our Mission?
As a Clinical Support Specialist, you will play a critical role in providing holistic, patient-centred care to Care Recipients in the community. You will work collaboratively with various Business Units and stakeholders to provide seamless care coordination for Care Recipients and their families to assess needs, develop care plans, and deliver healthcare services within the home and community setting. This position will be part of our Clinical team to plan, execute, and evaluate training activities to ensure the effective delivery of training programs to staff so that they are able to provide high quality clinical care.

What will you do…
Your focus will be on promoting health while supporting Care Recipients to achieve optimal well-being in their homes and community environments. You will also assist in ensuring best clinical practices and quality patient care are delivered to Care Recipients. In addition, you will:
Care Professional Training

  • Planning, executing and evaluating training activities ensuring the delivery of care is of high quality.
  • Ensuring the curriculum for training courses is up to date with current nursing standards, healthcare practices and so on.
  • Collaborating with other business units to identify, examine, plan, execute and evaluate clinical practices and training required
  • Provide specialist nursing advice and training to the other Homage teams as required.
Service Delivery Audits
  • Promote Clinical Practice and Standards
  • Review and validate competency-related policies and procedures regularly to ensure the delivery of safe, evidence-based clinical practice
  • Identify critical clinical skills for annual staff competency audit and clinical performance evaluation in line with organisational needs
  • Maintain accurate and up-to-date documentation of patients' assessments, care plan, interventions, and outcomes.
  • Adhere to documentation standards and regulatory requirements, ensuring confidentiality and privacy of the patient information.
  • Generate reports and communicate Care Recipient progress, trends, and concerns to Healthcare Business partners.
In Person Care Assessment (IPCA)
  • Conduct home care assessment and complete an initial assessment of client and family to determine home care needs, and provide a complete physical assessment and history of current and previous illness(es)
  • Develop individualised care plans based on patient goals, preferences and available community resources / care services
  • Collaborate with healthcare business partners to ensure coordinated and comprehensive care delivery
  • Excellent communication and customer service skills to build rapport with clients effortlessly
Clinical Incident Assessment and Performance Management:
  • Conduct thorough assessments of clinical incidents, identifying root causes and potential areas for improvement.
  • Collaborate with healthcare professionals and other relevant parties to ensure comprehensive and accurate incident analysis.
  • Utilise incident data to enhance training, policies, and carer support mechanisms.
  • Develop and oversee a comprehensive coaching program tailored to carer needs.
  • Create and implement innovative strategies aimed at increasing job satisfaction and performance levels.
  • Regularly review and refine these strategies to align with evolving carer needs and industry best practices.
Recruitment of care Professionals (Nurses):
  • Conduct interviews and assess candidates' qualifications, skills and compatibility with organisation's values.
  • Work with HR to ensure proper reference checks and background screenings are conducted for all candidates.
Skills & Experience you will need…
  • Minimum of 3-5 years nursing experiences essential
  • Strong clinical knowledge and understanding of community health practices, including case management and care coordination.
  • Strong experience in managing budgets
  • Up to 1 Year of Clinical Teaching (no formal certificate required)
  • Ability to operate independently with minimal supervision
  • Ability to excel in a fast-paced and rapidly changing environment
  • Excellent communication, interpersonal, written and critical-thinking skills.
  • Empathy, compassion, and ability to establish therapeutic relationships with Care Recipients and their families.
  • Current AHPRA Registration
  • Valid Police Check
  • Valid NDIS Screening Check
  • Valid Working with Children Check
  • Annual Flu Vaccination
  • Up-to-date COVID-19 Vaccination
  • Sound knowledge and understanding of relevant legislation surrounding Aged Care, NDIS and other community services
