Community Care Coordinator

We are currently seeking a suitably qualified and experienced applicant to fulfil the Community Care Coordinator role. The coordinator will need to provide excellent leadership and guidance to the members of the Community Care team to ensure the strategic development, coordinator and monitoring of a range of services in the Community Care area. This includes the Commonwealth Home Support Program, HACC PYP, Home Care Packages and NDIS within the Moyne Shire.

To be successful in this role you will need
Qualifications
Essential

A tertiary qualification in community services, health or an equivalent field.

A current driver licence, employee level working with children check and police clearance.

Experience
Essential

Extensive experience and knowledge in the delivery of the Commonwealth Home Support program, Home Care Package program, NDIS, and the Program for Young People.

Desirable

Experience in the delivery of programs within a local government environment is highly desirable.

Key selection criteria 1

Outstanding leadership and communication skills with the ability to develop and build productive relationships with a range of key internal and external stakeholders.

Key selection criteria 2

Proven skills in change management to implement actions arising from service reviews and other reforms as they arise.

Key selection criteria 3

Sound financial management experience including budget preparation, management of income and expenditure against budget and financial reporting.

Key selection criteria 4

Ability to prepare and present high quality written and verbal reports to a variety of audiences.
