Our Opportunity to you
The Community Connect Specialist plays a crucial role in effectively deploying clinical workers to clients in the community.
This hybrid role will be working out of our Parramatta/ local office and from home on a permanent part time or full-time basis.
The day to day
Providing quality administrative support to the allied health and nursing clinical teams, including reporting, document processing and client data management.
Supporting the clinical workforce through referral processing and allocation.
Responsible for document management, customer and referrer liaison, scheduling support of allied health and nursing services, and ongoing client case support throughout our customer’s journey.
Management and collaboration with new and existing service providers to deliver care to our clients in their home.

About you
Self-motivated medical and office administrator, skilled in customer service
Strong administration and time management skills, comfortable with using multiple systems
Experience in service delivery allocation and scheduling, along with effective case management and support skills
Effective communication skills and building rapport with external stakeholders
Adaptable and solution focused with the tenacity to overcome challenges
