What You Will Be Doing
The Community Connectors Program is an exciting new initiative modelled off the successful High Risk Accommodation Response (HRAR), a program that formerly worked to support vulnerable clients in high risk settings during the peak of the COVID pandemic. This new program is looking to employ a part-time, fixed-term Social Worker, Occupational Therapist (Grade 2) or Nurse (G3B) who will offer flexible and targeted mobile services within priority accommodation settings, engaging proprietors and residents to promote public health initiatives, identify strengths and needs and connect residents to local health and social services. The Community Connectors Program will provide assertive outreach to clients who are residing in low rise, high risk accommodation across the Frankston LGA. You will be required to build rapport with vulnerable clients, undertake needs assessments and complete service referrals, working within a social model of health framework and cognisant with crisis and brief intervention practices.
The successful applicant will work alongside a Senior Clinician (nurse or Allied Health Professional) to undertake joint visits within these accommodation settings. You will be required to undertake needs assessments, brief interventions and provide referral pathways for clients who are vulnerable and in crisis. Experience in housing, assertive outreach or community based work is desirable. You will attend meetings and a community of practice, comprised of other Community Connectors clinicians servicing LGAs across the state to share ideas and collaborate regarding sector development. You will also be responsible for statistical and data recording.

Selection Criteria Essential: • Tertiary qualification in Nursing, Social Work or Occupational Therapy
Essential: Registration with the Australian Health Professional Registration Board (AHPRA), or eligible to register with relevant professional association
Essential: Highly developed communication and documentation skills. Including negotiation, conflict resolution and creative problem solving
Essential: Demonstrated skills in fostering positive working relationships with team members
Essential: Exceptional time management skills and ability to work unsupervised in the community
Essential: Current drivers licence and willingness to transport clients as necessary
Essential: Proven ability in the management of clients with complex presentations and illness
