This role is based in Beaudesert which is in the beautiful Scenic Rim. We are located just 30 minutes from the centre of Logan, and 1 hour from Ipswich and the Gold Coast.

About the role
This role involves providing high quality care and support to individuals in the Scenic Rim and lower Logan areas, ensuring that their health and wellbeing are maintained. Whilst this is a full-time position, we are open to considering part time arrangements for the right person. Most of the work will be with our Aged Care clients, however you will also be able to provide support within our other program areas.
Duties will include:
- To provide nursing services in a community setting
- Completing initial assessments and on-going reassessments of clients needs to develop care plans, provide health assessments and implement health initiatives
- To work collaboratively with clients, their carers, family and/or advocate, Beaucare staff, health professionals and other external networks to maximise client independence and ability to remain living in their community
- To arrange and provide clinical care services, products and resources to support client care needs
- Providing information, advice and training to a multi-disciplinary team for medication management, personal care and simple wound management
- Building clinical networks with other organisations and allied health professionals
- To contribute to the development and continuous improvement of clinical systems, guidelines, policies and procedures to ensure compliance with relevant legislation and program guidelines
This role is paid under the Nurses Award as an Aged Care Employee Registered Nurse Classification Level 2. Pay point will be dependent on your experience.
Your skills and experience
- Demonstrated ability to provide high quality and innovative care
- Demonstrated knowledge and/or experience working with Home Care Packages and NDIS Care plans
- Experience in building clinical networks with other organisations and allied health professionals
- High level communication skills and interpersonal engagement
- Demonstrated experience in acquiring and updating knowledge to maintain compliance with legislation
- Experience working as a Community Nurse is desirable
Mandatory requirements
- Registered Nurse with current AHPRA registration (or eligible to obtain)
- NDIS Worker Screening Check (or eligible to obtain)
- Positive Notice Blue Card (or eligible to obtain)
- First Aid and CPR certificate
- COVID-19 Vaccination
- Driver's Licence
