About the role
You will play a crucial role in supporting Fathers and associated members to maintain good health and well-being in their preferred environment. Responsibilities include providing medical support, facilitating access to specialists, ensuring safe medication use, addressing health complications, and offering health education. You will also report on care provided and attend national association meetings. The role involves active case management, promoting quality of life, and navigating the aged care system to achieve client outcomes. You will lead teams, promote dignity and values, and ensure compliance with organisational policies and legislation.

- Strong communication and a genuine passion for providing care to the elderly.
- Excellent abilities to work independently and as part of a team.
- Attention to detail, a proactive approach to problem-solving, and a dedication to continuous improvement.
- Ideally you will have expertise in Care Link, case management, community services, care planning, spreadsheets, My Aged Care, home care packages, CHSP funding, catheters, stomas, and continence management.
