We are currently expanding our business and we are looking for Occupational Therapists (OT) experienced in Equipment Prescription and Home Modifications to support our clients on full time or part time basis from our Busselton base and also covering Margaret River. We also have a role based from our Bunbury base. This role provides the flexibility of seeing clients in the community and working from home. In this role, you will be responsible for the delivery and coordination of Occupational Therapy services to achieve improved occupational, social and health outcomes, optimum independence and improved quality of life for our clients.
Working in a community aged care setting, you will work independently completing home and community-based assessments and provide appropriate recommendations, intervention, and evaluation. You will be working with clients from the Commonwealth Home Support Programs (CHSP) and Home Care Packages (HCP). You will be provided with a Smart Phone, Laptop and access to a fleet vehicle for home visiting.

Key Responsibilities- Community based role with a strong focus on enhancing the independence of our clients
- Plan and carry out required services to clients following guidelines for assessment and treatment required
- Maintain a close liaison with other community resources to ensure the ongoing care
- Liaise with families/care providers by providing ongoing support, counselling and education
- The ability to work independently
- Plan and carry out relevant training to staff
- Prepare reports as required
Under the Aged Care act 1997, successful candidates will be required to provide a satisfactory National Police Clearance prior to commencement.
About youTo be considered for this role, you will meet the following criteria:
- Tertiary qualifications in Occupational Therapy
- Current registration with Australian Health Practitioner Regulation Agency (AHPRA)
- Previous experience working as an OT in a community setting
- Ability to identify and provide appropriate advice on complex care needs by exercising initiative and innovation in the application of professional practices under limited direction.
- Previous experience in minor home modifications and equipment prescription
- Ability to use a reablement/restorative model of care to enhance the independence of our clients
- Excellent interpersonal and communication skills (written and verbal)
- High level computer skills
- Ability to work autonomously as well as work well within a multi-disciplinary team
- Highly organised, strong attention to detail and ability to meet deadlines
- Valid Australian drivers licence
