We have an exciting opportunity to join our Community Care Team located in the beautiful Barossa Valley.
The Community Registered Nurse position contributes to the implementation and maintenance of high standards of Community Home Care. The philosophy, policy and goals of the Barossa Village Board, the Aged Care Act, Home Care Package Guidelines and Aged Care Quality Standards provide the context and parameters for service delivery for Community Services Clients.
Responsibilities:
- Undertake clinical/nursing assessments and development of the clinical components of person-centred care plans.
- Consult with the Manager for any client concerns needing follow up action/risk assessment.
- Update systems and documentation as per organisation and legislative requirements.
- Promotion of wound prevention and wound healing through the utilisation of clinical knowledge and experience to undertake holistic assessments.
- Provision of support, direction for medication management and care related tasks, e.g. pressure stockings and wound management.
- Ordering any recommended clinical supplies / equipment and ensuring it is the within clients budget.
- In conjunction with Care Coordinators develop care plans.
- Communicate with General Practitioners and Allied Health professionals as required and in consultation with the respective Clients and Community Care Team.
To be successful in these roles you will need:
- Registration with APHRA and current practicing certificate.
- Experience in providing nursing care to older people.
- Experience in coordination of Community Home Care Client care.
- Computer competency to enable accurate and timely development and maintenance of Client documentation and records.
- Current Drivers Licence
- Participate in Barossa Village annual mandatory vaccination programs
- Have current National Criminal History Check / Clearance
