Position Overview

We are looking for dedicated community Support Workers to provide high-quality, person-centered support to NDIS Participants. Your role will be crucial in helping them achieve their goals, enhance their quality of life, and improve their physical and mental health.
About the Position
The Support Worker will provide support with people living with complex Mental Health, and psycho-social challenges and needs aligned with their individual support plans.
Working with external care teams and families, the role is responsible for providing high-quality, responsive, and person-centered support to assist participants enhance their everyday living skills, gain access and participation in community to achieve their goals, enhance their quality of life and physical and mental health.
Key Responsibilities
- Individualised Support: Pay attention to and generate opportunities and activities that align with the interests and objectives of individuals.
- Knowledge and Care: Demonstrating an understanding of health and well-being, cultural plans, transition plans, and behavior support plans. You will also be knowledgeable about protocols regarding restrictive practices and medication requirements.
- Community Inclusion: Assisting Participants engaging in community activities and services based on their preferences and abilities. You will help them establish and maintain relationships and interests.
- Safety and Wellbeing: Demonstrating a strong commitment to the safety and well-being of yourself, Participants, and others in the work environment.
Selection Criteria
- 2 Years experience working in the human services field.
- Current Driving Licence with own vehicle.
- Ability to meet deadlines, work with minimal supervision and pressure.
- Understanding of the challenges faced by people living with mental health issues.
- Effective written and verbal communication skills with the ability to effectively use Microsoft Office and case notes
Desirable :
- Certificate 3 in Community Services or related field
Your Application for this position will require you to have:
- NDIS Worker Screening Clearance (obtained from Service NSW)
- Current First Aid Certificate
- Unrestricted Australian Driver’s Licence
- Covid Vaccination Certificate (strongly encouraged)
