Summary of position:
We are looking for an enthusiastic person to join our team as a Community Team Leader in a growing and rewarding organisation.

About the role:
The Team Leader supports the Community Manager to ensure that the Barunga Village Community Program operates efficiently and effectively and provides a high quality of client service which is in line with Barunga Village policies and procedures, meets or exceeds the Aged Care Quality Standards and is within legislative guidelines, and meets agreed budgeted targets.
The Team Leader manages and leads the day-to-day operations of the Community Program. The role is the line manager for all client-facing staff, and supervises and manages client support services.

Responsibilities of the role:
Staffing
Monitor and identify recruitment/rostering requirements and advise the Community Manager when additional staff or adjustments are required.
Support the Community Manager in staff recruitment, onboarding and development staff.
Foster and support a hub-and-spoke approach where staff and clients are grouped according to their location.
Foster and support a client team approach, where individual clients allocated a designated team of support workers
Manage staff performance for all direct reports, including but not limited to probation period reviews, annual performance program, counselling and disciplinary actions.
Refer to Community Manager any significant or ongoing staff performance or development issues.
Refer any issues regarding Contractor management to Community Manager
New clients
Accept referrals for prospective clients from either the portal, the Enquires and Admissions Officer or Community Manager.
Allocate to a Client Liaison Officer (CLO), using a hub-and spoke approach
Existing clients
Act as first point of contact/support for CLOs regarding client support issues.
Ensure intake, assessment, care planning, review and referral processes are in place as per Barunga Village policies and procedures
Ensure client agreements, billing and invoicing processes are in place and are in accordance with Barunga Village policy procedures and legislative requirements.
In consultation with CLO, refer clients to Community Nurse for clinical assessment as indicated
Monitor and where necessary, support the CLO to manage minor and informal complaints, incidents and feedback.
Refer all clinical incidents to Registered Nurse for investigation and management.
Manage all formal or moderate level complaints, feedback and incidents investigation and follow up, in consultation with client and with CLO. Meet with individual clients where indicated, to address their issues/concerns.
Ensure Open Disclosure is conducted in line with Barunga Village policies and procedures and legislative requirements.
Escalate to Community Manager any significant/ongoing feedback or incidents, including SIRS incidents.
Communications
Ensure effective communication to clients and their representatives, through regular face to face meetings, events and written communications.
Communicate effectively with staff including through regular in person staff meetings, events and communications,
Provide monthly written operational reports to Community Manager
Assist Community Manager, when requested, to develop written consumer and stakeholder communications
Conduct individual staff wellbeing checks at least quarterly.
Act as an advocate and facilitator for clients.
Respect clients’ cultural beliefs and right to privacy, ensuring confidentiality at all times.
Share in the on-call after hours roster (for staffing and scheduling issues only).
Departmental and organisational involvement
Plan, participate and attend meetings as required and as directed by Community Manager
Participate in strategic planning processes as directed by Community Manager
Manual Handling Champion responsibilities
Be conscious of the own manual handling postures to ensure own safety and to set an example for others on how to perform the tasks in a safely manner.
Carry out the allocated number of manual handling competency assessments with employees.
Provide constructive feedback to colleagues and team members about their manual handling posture in a positive way and using appropriate language.
Discuss with others the correct technique to improve manual handling postures.
Facilitate team members learning of key principles and how apply them to the work tasks.
Assist the manager to identify manual handling hazards.
Carry out risk assessments and identify appropriate controls as outlined in the Risk Management Policy.
Any other duties commensurate with the employee’s skills and experience as directed by the Community Manager.
Work, health and safety
Adhere to defined work, health and safety policies and procedures related to the work being undertaken in order to ensure own safety and that of others in the workplace.
Reporting of any possible hazards in the workplace and community.
Reporting of any ‘near misses’ and participation in incident/accident investigations as necessary.

Support the Community team in opportunities of growth.
Obtain orientation in all areas as specified on commencement of employment.
Conduct and participate in the performance appraisal review process.
Undertake further education as appropriate and participate in in-service and online training as provided by Barunga Village.
Quality improvement
Recognise the need and participate in quality activity programs relevant to the area.
Be familiar with and strive to achieve the standards, requirements and criteria of the Aged Care Quality and Safety Commission’s Quality Standards.
Actively participate in the quality review process to ensure compliance.
Abide by the policies and procedures of Barunga Village.
Other
Assist in the promotion of the philosophy and aims of Barunga Village.
Any other duties commensurate with the employee’s skills and experience as directed by the Community Manager.
About you:
Diploma of Community Services or higher.
Experience in working in a community environment or working with the aged.
Experience in managing a staff team.
Desirable qualifications and experience
Knowledge of Aged Care Quality Standards.
Knowledge of the accreditation process is desirable.
Experience in the nursing process or case management.
Experience in the development of individualized care planning.
Knowledge of aged care and community services with a focus on reablement and consumer directed care.
Knowledge of Occupational Health, Safety and Welfare, Equal Employment Opportunity and quality improvement.
Understanding of dementia and the ageing process/and related diseases
Core competencies

Conditions
Must provide a NDIS worker screening clearance that is dated not more than five years old to enable the assessment of whether the employee is suitable to provide care under the Aged Care Act. This clearance must remain updated by the employee during the contract period.
Must comply with having annual flu vaccination and be fully immunised against COVID-19 (including booster).
Must sign a statutory declaration to identify that have never committed any serious criminal activity outside of Australia.
Must not be included on the aged care banning order register.
Must comply with Barunga Village’s Code of Conduct.
Special conditions
Willingness to travel as required to cover all service region.
Some out of hours work may be required.
Must have a smartphone.
Must have current driver’s licence and reliable vehicle.
