Seeking an experienced Community worker Case manger

We seek to employ reliable and experienced full-time community work Case manager to join our established team.
The responsibilities include:
- Assessing clients' needs and planning, developing and implementing educational, training and support programs
- Interviewing clients and assessing the nature and extent of difficulties
- Referring clients to agencies that can provide additional help
- Assessing community needs and resources for health, welfare, housing, employment, training and other facilities and services and reporting on the progress of clients
- Liaising with community groups, welfare agencies and government bodies
- Supervising offenders on probation and parole
- Assisting young people to solve social, emotional and financial problems
- Preparing submissions for funding and resources and reports to government bodies and other agencies
- One-on-one client facing work including medication assistance and basic personal care.
- Assist in new client sign-ups by providing appropriate information to prospective clients.
- Participate in project work as directed by the organisations senior management.
- Complete, maintain and store documentation in a confidential manner.

The successful applicant will have:
- Associate Diploma in Community Services or be studying towards such a qualification;
- A minimum of 2 years of work experience as a Case manager within the Community Services sector.
- Have a willingness to learn on the job.

- Effective teamwork skills
- Accurate and timely decision-making skills
- Ability to work in a fast-paced environment.
- Solid understanding of Outlook, Excel, Word and Accessing Apps on your mobile phone
Licence/Certification:
- Driver Licence (Preferred)
- Working With Children (WWC) Check (Preferred)
- National Police Certificate (Preferred)
- NDIS Worker Screening Check (Preferred)
Work Authorisation:
- Australia (Preferred)
