Community Worker - Case Manager

Seeking an experienced Community worker Case manger

We seek to employ reliable and experienced full-time community work Case manager to join our established team. 

The responsibilities include: 

• Assessing clients' needs and planning, developing and implementing educational, training and support programs
• Interviewing clients and assessing the nature and extent of difficulties
• Referring clients to agencies that can provide additional help
• Assessing community needs and resources for health, welfare, housing, employment, training and other facilities and services and reporting on the progress of clients
• Liaising with community groups, welfare agencies and government bodies 
• Supervising offenders on probation and parole
• Assisting young people to solve social, emotional and financial problems
• Preparing submissions for funding and resources and reports to government bodies and other agencies
• One-on-one client facing work including medication assistance and basic personal care.
• Assist in new client sign-ups by providing appropriate information to prospective clients.
• Participate in project work as directed by the organisations senior management.
• Complete, maintain and store documentation in a confidential manner.

The successful applicant will have:

• Associate Diploma in Community Services or be studying towards such a qualification;  
• A minimum of 2 years of work experience as a Case manager within the Community Services sector.
• Have a willingness to learn on the job.

• Effective teamwork skills
• Accurate and timely decision-making skills
• Ability to work in a fast-paced environment.
• Solid understanding of Outlook, Excel, Word and Accessing Apps on your mobile phone 

Licence/Certification:

• Driver Licence (Preferred)
• Working With Children (WWC) Check (Preferred)
• National Police Certificate (Preferred)
• NDIS Worker Screening Check (Preferred)

Work Authorisation:

• Australia (Preferred)
