Continuity Care has multiple roles available for Care Workers with experience in the delivery of supports to people living with Complex Care and behavioural support needs. Due to client preference, this role is only available to female applicants.

We recognise the need for culturally appropriate support and endeavour to provide this as effectively as we can, utilising the resources available to us to ensure everyone’s needs are addressed. The support we provide is individualised, self-directed and integrated.
The role:
- Delivery of 1:1 and 2:1 support to an individual with complex support needs and challenging behaviours.
- Ongoing implementation of positive behaviour support strategies and use of restrictive practice.
- Providing personal care support including medication assistance.
- Providing domestic assistance, including meal preparation, that encourages participation where possible.
- Proactive planning of daily activities that supports skills development relevant to individual needs and goals.
- Supporting engagement in community-based activities.
- A minimum of 4 shifts per week with combination of day, afternoon and active overnight shifts.
About You:
- Minimum 2 years' experience working with Complex Care needs and challenging behaviours
- Minimum Cert III or IV in Individual Support, Community Services, Mental Health or other relevant field
- Experience in Positive Behaviour Support and Restrictive Practice.
- A passion for working with people with complex support needs and the ability to hold unconditional positive regard.
- Understanding and commitment to a person-centred model of practice.
- Current Blue Card and NDIS Worker Screening Card.
- Current Australian Driver's Licence and regular access to a comprehensively insured vehicle.
- Flexibility to work a combination of days, nights, active overnights and weekends.
