The Role We’re currently looking for a Connections Transitions Coordinator aims to improve continuity of care for patients of the correctional centres with Drug and Alcohol problems, who are being released to the community. The Connections Program works within an assertive linkage model that aims to link patients with relevant health and welfare service providers post release.
The Connections Program team utilises a broad array of contacts, both in the correctional and community environments to assertively link patients into services appropriate to their individual needs.
