About the role:

An exciting opportunity now exists for a Credentialed Diabetes Educator to join the Quality Pharmacy Group for our northern store locations.

As a Diabetes Educator at our Quality Pharmacy, you will be a key player within a multidisciplinary team that thrives on collaboration and passion. Our team works cohesively, drawing on each other's expertise to deliver unparalleled outcomes for our patients.

In this role, you will be at the forefront of diabetes care, working alongside a diverse team of professionals, all dedicated to enhancing the health and well-being of those impacted by diabetes.

Your role will primarily focus on working hand-in-hand with non-acute clients, ensuring they receive holistic care tailored to their needs. By liaising and coordinating with other health professionals in our clinic, you will be instrumental in ensuring that every patient benefits from our integrated approach to diabetes care.

Key Responsibilities:

• Seamlessly integrate diabetes self-management education into clinical care.
• Liaising with doctors and allied health professionals to increase diabetes education awareness 
• Leverage group education to meet individual patient needs.
• Educating pharmacy team on diabetes 
• Design and implement effective self-management plans to boost health results.
• Promote awareness on health, disease, disability, and other pertinent health topics.

About You? 

• AHPRA Registration
• Current ADEA Membership
• Legal work status in Australia.
• Relevant qualification as a Credentialed Diabetes Educator with currency of practice
