About the Role
The Credentialled Diabetes Educator is integral to the AWH Community Health Team, valued for their expertise in diabetes education. The successful candidate will serve as a specialist primary care practitioner, providing education and expert clinical guidance to clients, service providers, and colleagues within AWH. Practice is grounded in the principles of a client centered approach to the provision of diabetes self-management.
Credentialing through ADEA is preferred. However, those working towards credentialling are encouraged to apply and will receive support through a dedicated learning pathway. Employment Type: Permanent Full-Time position available, with the option to consider Part-Time or Job Share arrangements.
