To be successful in the role, you will be a credentialled Diabetes Nurse Educator and have demonstrated knowledge and experience working with a chronic care framework and self-management support. There may also be an opportunity in the future to work in a nurse-led chronic disease management program.
About the role:
The position of Diabetes Nurse Educator (DNE) at Inspiro assists clients, families and carers gain the information, knowledge, skills, motivation and confidence they need to manage their diabetes. The DNE provides clinical assessments, diabetes specific information and knowledge, self-management education and support, designed to empower clients to manage their own health.
A key component of this role is involvement in the IDEAs clinic, a partnership between Eastern Health and the eastern region Community Health services, which provides an integrated, interdisciplinary chronic disease management service for people with Type 2 diabetes. Through the IDEAS services, this role will work closely with the Eastern Health Endocrinologists to support more complex clients to manage their diabetes.
Position Responsibilities:
In the role you will:
- Undertake comprehensive assessment of clients presenting with diabetes
- Deliver education that is culturally appropriate, evidence-based and informed by advanced level of clinical knowledge and problem solving
- Develop care plans with clients that reflect their needs whilst aiming to promote independence and enhanced quality of life and monitor their progress
- Initiate Insulin stabilisation for clients as required in collaboration with the General Practitioner and/or Medical Specialist. Assist clients to manage the process associated with commencing Insulin and stabilisation
- Support and coordinate the care for clients presenting with chronic disease/s under the Integrated Chronic Disease Management (ICDM) program.
About You:
- Credentialled Diabetes Nurse Educator with current registration with Australian Health Practitioner Regulation Agency (AHPRA) and Australian Diabetes Educators Association (DEA)
- Eligibility for registration as an MBS service provider
- Demonstrated experience and skill in a wide range of clinical assessment procedures, treatments and activities
- Demonstrated experience in self-management support.

- hold an Employee Working with Children Check, and
- meet the mandatory COVID-19 vaccination requirements.
