• Collaborate with healthcare professionals to develop personalized diabetes management plans
• Empower patients by providing education on nutrition, exercise, and medication management
• Conduct group sessions and one-on-one consultations to support patients in achieving their health goals

Preferred Requirements:

• Demonstrated experience in diabetes education or related healthcare field
• Strong communication and interpersonal skills to connect with patients from diverse backgrounds
• Ability to work collaboratively within a multidisciplinary team
• Experience in developing and delivering educational materials
• Understanding of cultural, social, and economic factors affecting diabetes management

Preferred Qualifications:

• Bachelor's degree in Nursing, Dietetics, or related field
• Certification as a Diabetes Educator (CDE) highly preferred
• Current registration with relevant professional board or association
