Job description

Where you'll be working:
This position will have a shared location of Forster Community Health Service and Taree Community Health Service, working within the Generalist Care multi-disciplinary team, including Palliative Care Nursing, Cardiac & Respiratory Rehabilitation, Continence, Dietetics, Diabetes, Wound Management, Immunisation and Audiometry. You will also provide outreach services to Bulahdelah and Gloucester Community Health Centres.

About the role:
You will be providing a comprehensive Diabetes Educator service to patients from the community, including paediatrics and adults, in line with the current admission criteria, working collaboratively with the Community Dietetics service, visiting Endocrinologist and members of the Diabetes in Pregnancy team.

You will be supported by management to attend education and training opportunities that will build the clinicians skills and knowledge to help support patients, carers and other health professionals.

About you:
We are searching for an experienced clinical professional who has sound knowledge and experience in diabetes to provide clinical services to patient groups and circumstances of a complex nature requiring advanced practice skills and who will exercise independent professional judgement.

You will have excellent communication and interpersonal skills with the ability to engage with others and communicate effectively with a range of stakeholders and ensure patients and their families are well supported and informed of clinical interventions and service delivery processes, enabling them to make informed decisions relevant to their health needs.

Requirements:

  • Degree qualified as a Physiotherapist, Dietitian, Pharmacist and Registered Nurse with current registration with AHPRA where relevant and associated membership of relevant professional board.
  • Completed an Accredited Australian Diabetes Educator’s Course with current credentialing.
  • Eligibility to drive in NSW and willingness to travel for work purposes which may involve driving long distances.
  • To be eligible for appointment as Pharmacist grade 2, candidate must be able to demonstrate fulfilment of the criteria for grade 2 pharmacist in accordance with the Health Employees' Pharmacists (State) Award.
  • To be eligible for appointment as Clinical Nurse Specialist Grade 2: Post registration qualifications relevant to the area of speciality with at least 3 years post registration experience within the clinical area of specialty.
