Position Summary
Dietitian - Grade 2
Community Rehabilitation Program (CRP)

• Yarra Ranges Health, Eastern Health
• Permanent part-time – 2 days per week
• Friendly & supportive team

You will be an enthusiastic and innovative grade two dietitian who would like to join a very dedicated interdisciplinary allied health team in the Community Rehabilitation Program (CRP) at Eastern Health. You will be doing a mixture of home and centre based sessions as well as want to embrace the use of Telehealth.
To be successful in the role:

• Experience in the area of physical rehabilitation (highly desirable)
• Strong commitment to high-quality patient care
• Well-developed interpersonal skills
• Clinical skills in managing a complex caseload using a client-centred approach.
• High-level communication skills, both written and oral, to enable the development of a good rapport with clients and staff
• The organisational skills of a self-starter able to administrate a defined caseload
• Demonstrated ability to participate in innovative practice and quality improvement

You will be required to:

• Plan and implement client centred occupation focussed rehabilitation
• Deliver a high quality, accessible and coordinated service to our clients both in the client’s home and in the centre.
• Be an active and engaged member of the interdisciplinary team
• Meet the performance requirements as set out by the CRP Team Leader
• Excellent communication and organisational skills
