What you'll be doing
The Disability Navigator is responsible for coordination of effective clinical and operational processes that support staff in the timely commencement and progressing of NDIS applications for inpatients, so as to minimize lengths of stay associated with the NDIS approval and planning process.
The Disability Navigator will assist staff in navigating and implementing relevant resources (internal and external) that support NDIS access, planning and escalation requirements, as well supporting the development of NDIS Champions to build NDIS capability across Western Sydney Local Health District.
The Disability Navigator will assist to improve the care experience for people with disability, their families and informal supports by facilitating continuity, coordination and transfer of care between admitted and community settings.
The Disability Navigator will provide guidance, leadership and advice to facilities to support the needs of people with disabilities who present to hospital in collaboration with the NDIS and SASH/ComPacks Coordinator.

Essential Criteria:
Qualification: Tertiary qualifications for relevant profession. Social Worker – Must hold as a minimum a bachelor’s degree in social work which provides eligibility for membership of the Australian Association of Social Workers.
Registration: General registration with AHPRA (for relevant professions)
Selection Criteria: * Relevant tertiary qualifications and relevant clinical experience as an Allied Health Clinician Level 3 as required by the health professionals state award.
- Demonstrated advanced clinical reasoning, experience and practice skills as relevant to assessment and treatment of people with a disability and/or functional impairment, including a high level understanding of the NDIS.
- Demonstrated ability to apply advanced clinical reasoning skills, exercise independent professional judgment and operate autonomously with minimum direct clinical supervision.
- Demonstrated ability to work with a broad range of internal and external stakeholders to resolve complex issues impacting on the safety and independent functioning of patients.
- Experience coordinating/navigating care for people with complex needs (including where behaviours of concern are present) across multiple services and agencies and the understanding of applying reasonable adjustments in the hospital setting.
- Demonstrated experience of effective negotiation, collaboration and communication skills with a wide range of stakeholders including presenting information to a wide audience.
- Hold a current driver’s license valid within Australia
