Disability Support Mentor (Ndis) - Collie

Our Disability Support Mentors are person-centred and passionate about working with people with disability to help them achieve their goals in ways that are meaningful to them.  You assist in building capacity of our participants and enable them to access and enjoy the benefits of employment, you’re their mentor, and coach - you make a real difference, every day.

What are the Primary Objectives of the Role?

• To use initiative and work both independently and in a collaborative team environment
• To support participants in identifying and creating self-determined goals.  
• Working with participants to understand areas of development required to build capacity within work skills including support needs.
• Applying knowledge learnt about our participants (e.g. their interests and skills) during the Discovery Process to then explore options of Customised Employment steps and activities.
• Build and maintain effective relationships with participants, employers and the wider community and help build awareness of NDIS Supports in Employment, NDIS Employment Supports and NDIS School Leaver Employment Supports (SLES.
• Safely transport participants as required.
• Be an ambassador for Forrest Personnel in the community and awareness of our NDIS.

To be successful in this role, ideally you will have:

Qualifications

   Essential

• NDIS Clearance (or willing to obtain)
• NDIS Worker Orientation module (or willing to obtain)
• National Police Clearance (within the last 3 months)
• Current WWC (or willing to obtain)
• Current Drivers Licence
• Own a vehicle

Experience

• Previous experience in a similar role (desirable)
• Demonstrate the ability to work in a positive and collaborative manner with colleagues.
