As a Support Worker, your primary duty will be to improve the lives of those within your care by implementing training and support services aimed at maximising the living, social and recreational skills of people with a disability.
Core objectives include:
• Support clients to participate in age appropriate activities and tasks including in home support and community access.
• Support clients to access activities and outings in the community.
• Provide assistance to clients with personal care, social and medical needs and with day to day living; such as banking, attending appointments, meal preparation, washing, cleaning toileting, showering and shopping.
• Provide direct care assistance to each client in accordance with their service plan and as directed by the House Leader.
• Work in accordance with the medication administration policy, work health and safety policy and procedures and privacy and confidentiality.
• Monitor the safety and wellbeing of the clients and report any concerns to the House Leader.
• Actively contribute to a team environment through open communication, participation in regular staff meetings.
• Assisting in daily planning, advocacy, communication and transport as required by the service or the people we support in both the participants home or within the community.
• Adhere to all policies and procedures.
