Care & Connects was founded with the primary goal of enhancing the lives of individuals with disabilities through genuine care and connection. Our core philosophy is prioritising the unique needs and preferences of each participant through actively listening, building on their strengths, and ensuring that our support is reflective of a person-centered approach.
Care & Connects is an innovative NDIS provider who is dedicated to delivering the best quality support to each of our participants. To assist with this, we have a dedicated Psychosocial Disability program led by experienced Mental Health professionals, delivering Supported Independent Living (SIL) and Community Participation (CP) supports across the Adelaide region. We also have a separate Disability Program for those who need more assistance with daily activities and physical support needs.
Care & Connects currently have vacancies across our Psychosocial and Disability programs for passionate, experienced Support Workers who are interested in developing their careers. About you:
We are looking for compassionate individuals with a genuine desire for supporting individuals with specific conditions and needs. You possess a strong understanding of the NDIS framework and are dedicated to its principles.
Qualifications
- Minimum Certificate 3 in Individual Support (Disability)
- Medication Administration Accreditation
- NDIS Worker Screening
- Manual Handling Training
- First Aid and CPR
- Any additional trainings such as PEG/Bowel/Diabetes is an advantage
Selection Criteria
- Availability to work passive & active nights
- Available to work across a range of locations including Eastern, North Eastern and Northern Adelaide. Opportunities in Southern Adelaide region coming soon!
- Minimum 1 year in high care roles, including but not limited to high needs care, etc.
- An understanding of the impact chronic pain, trauma, grief and loss has on an individual and their families, particularly those with complex needs.
Your tasks may include but not be limited to the following:
- Providing personalized care and support to NDIS participants, including personal hygiene; meal preparation; medication; and activities of daily living.
- Implementing positive behaviour support plans and crisis intervention strategies
- Provide emotional and practical support to participants according to their individual needs.
- Monitor participant’s progress and report any changes or concerns as required
- Maintain accurate records and documentation in line with NDIS and organisational requirements
