About Creative Community Pathways:
Established in 2020, Creative Community pathways is dedicated to empowering individuals to live a quality of life. With a foundation of over 20 years of deep industry experience in disability and Specialist knowledge Behaviour Support we are committed to outcomes and truly make a difference in people's lives.
We are now seeking a range of Support workers with an ambition to make a difference and focus on outcomes for our complex participants in the North and West of Melbourne and join our innovative team in Melbourne.
It is our purpose to increase the quality of life for people with a disability and will roll out our skill development program in 2024. We are looking for a team who is willing to grow with us and raise the quality of services delivered within the NDIS framework.
The Role:
In this exciting role, you will be pivotal in showing up in a loyal and consistent manner where you have a thorough understanding of your participants’ needs. You are responsive to the needs of our participants and can count on a team who has your back.
Creative Community Pathways aims to provide a regular roster, with a request to be flexible when changes arise. We ask you to use open communication and a drive for personal growth and high-quality results.
Key responsibilities include:
- Show initiative: Support people with a disability to have
the best life they possibly can, whilst experiencing their full human rights and feeling valued as part of their local community. You have a natural curiosity for increasing the quality of a clients’ services. - Teamwork: Actively liaise with a multidisciplinary team to provide a high-quality service focused on participant and their network of supports and priorities delivered within both universal and specialist support services.
- Leading with Excellence: Be open to growth and implement positive development programs for participants whilst respecting health and safety for the participant and yourself.
- Agile and Effective Communication: Rapidly respond to client and team communications, managing expectations and adhere to all reporting requirements set out.
- Consistency: Show up in a consistent manner and follow the behaviour support plan, communication agreements that have been agreed on for your participant.
Who We Are Looking For:
You are a natural leader, keen on understanding your participants needs and ensuring they receive a quality service where choices and wins are celebrated!
You have an eagerness to learn and develop yourself and you are open to feedback and staff training. You connect well with your participant, and you are comfortable to work towards their individual goals. You are aware of signs and triggers; you can think on your feet and show a proactive approach making sure everybody is safe and well.
A Day in the Life:
As a Disability Support Worker at Creative Community Pathways, we aim to provide you with a regular roster of participants who you have connected well with. Your skills, personality and location are carefully matched to the participant. Each day you show up on time and you are aware of the care plan and behaviour support plan of your participant.
You can think on your feet and respond in a proactive manner to the participants’ presentation. The relationship you have built with the participant offers opportunities for their growth and you could offer new locations they may find joy in or engage in a skills development program. You see the importance of daily reporting and you show compliance in completing any charts to assist the participants’ future funding requests.
Your expertise how to assess risks in the community and communicate openly with your team is crucial to ensure everybody is safe and you experience the team has your back.
Key Qualifications:
- Demonstrated experience (2-5 years) in Disability work, preferably with participants with Complex needs (nonverbal, ASD, mental health).
- Cert IV Qualifications in health, education, or community services.
- Exceptional verbal and written communication skills to provide written outcomes and data to support any future planning and training.
- Ability to implement Behaviour support plans and care plans, maximising choice, and control for participants.
- Proven self-motivation and the ability to operate independently with high standards.
