About the role
We are seeking the service of an experienced Disability Supports Facilitator to join our Gold Coast team. This is a work from home position with flexible hours starting as part time with the possibility of going to full time.
The Disability Supports Facilitator will be the link between our NDIS participants, our disability support workers and their disability service providers. The Disability Supports Facilitator will need to develop strong relationships and must be able to provide service which exceeds accreditation standards.
The Disability Supports Facilitator will also be responsible for:
- Managing supports to participants.
- Making referrals to relevant service providers to address participant concerns and coordinate the participant’s care as part of a multidisciplinary team.
- Maintaining accurate and timely case notes and reporting on participants’ progress.
- Providing effective leadership to employees under your supervision.
- Ensure rostering and allocation of staff is actioned.
Skills and experience
- Demonstrated knowledge and sound understanding of the National Disability Insurance Scheme and what it means for participants.
- Demonstrated organisation skills to effectively manage workload including complex rostering, caseload management ensuring participant satisfaction.
- Ability to work autonomously and as part of a team.
- High level of interpersonal skills including the ability to develop and maintain professional relationships within a team, with participants, across service providers, and with various stakeholders.
- Professional and compassionate verbal and written communication skills.
- Demonstrated experience supporting participants and managing teams for participants who have complex medical needs including, but not limited to spinal injuries, acquired brain injury and neurological disorders.
- High level of problem-solving skills, to successfully identify problems, develop solutions, and implement these using a logical and systematic approach.
- Demonstrated competence in computer applications, i.e. Microsoft Word, Excel PowerPoint and other applications relevant to the role to ensure work is completed in a professional, accurate and timely manner.
The successful applicant must have their own car, current National Police Clearance and NDIS workers screening check. The applicant must have the legal right to work in Australia.
