About the role
We are seeking Discharge Coordinators to join our team in providing a highly responsive and flexible service to support consumers on their recovery journey . The roles will supporting acute inpatient units with expert advice and guidance to support care planning and decision making in order to engage the most appropriate ongoing community service supports for consumers following inpatient care. The Discharge Coordinator plays an essential role in the team's ability to provide high quality services within a recovery oriented framework. If you are keen to provide responsive, flexible and professional planning advice, linkage and support to consumers, carers and service providers, then this is the role for you.
About you
To be successful in this role you will possess:
- Relevant registration for your discipline/Current registration with AHPRA or eligible for membership of AASW
- Sound theoretical knowledge of clinical assessment and interventions in Mental Health.
- Extensive knowledge of community Mental Health care and support services
- Demonstrated commitment to and ability to practice utilising the recovery approach in Mental Health treatment and support
- A strong commitment to high quality care
- The ability to work as part of a diverse team
- Excellent communication, IT and organisational skills
- Highly developed interpersonal, negotiation skills and leadership experience
