Job description

Discharge Planner delivers complex specialist nursing care and leads the development of specialist clinical practice and service delivery to provide effective care for patients and clients with complex care needs.

What you'll be doing

The NSLHD Nursing and Midwifery Workforce will be compassionate, competent, confident, contemporary, capable and committed to leading person-centred health care.


The Discharge Planner forms part of the hospital’s patient flow, Sustainable Access &Capacity Management team. The Discharge Planner facilitates the efficient &effective coordination of the discharge process as per PD2011_015 Care Coordination:Planning from Admission to Transfer of care in NSW Public Hospitals. This is to improve the patient experience, access to care and patient flow within the hospital setting. 
The position of Discharge Planner works closely with the PFM &DDON to coordinate the resources that operationally impact on patient flow from inpatient admission to discharge facilitation; to implement practices that improve length of stay;and capacity management.

People of Aboriginal and/or Torres Strait Islander background encouraged to apply.Candidates will need to meet the following criteria:

  1. Current registration as a Registered Nurse – Division 1 with Australian Health Practitioner Regulation Authority (AHRPA). Relevant post registration qualifications and at least 3 years’ experience working in the clinical area of their specified post-graduate qualifications or such other qualifications or relevant equivalent experience.
  2. Demonstrated knowledge & ability to mentor & coach nursing staff to enhance professional development. Demonstrated ability to work individually & as part of a multidisciplinary team.
  3. Demonstrated experience in leading & initiating activities to improve clinical service delivery & patient clinical outcomes.
  4. Demonstrated understanding of operational factors that influence patient access & improve the patient journey inclusive of NSW Government policy frameworks for health reform; and that impact on patient access & capacity management.
  5. Proven leadership qualities & the ability to set priorities, establish & achieve agreed tasks & plans together with the ability to work with a broad range of health disciplines.
  6. Demonstrated skills in computer use & applications such as Windows; Patient Flow Portal & Microsoft Office desktop applications.
