Role Description
The Elder Care Support Coordinator, in collaboration with the Manger, Care Coordinator will be responsible for the coordination of the Elder Carer Support Program, ensuring cultural integrity, cultural safety, and trauma informed practice and high standards of care for clients. The Elder Care Support Coordinator will lead a team of Aged Care Connectors to support identified clients to navigate access to appropriate local and remote aged Care Support services. This role will also liaise with aged care Assessors to ensure clients are well-supported throughout the assessment process.
To be considered for this position, you will require:
- Diploma level qualification in aged care, disability, social, community services, health or related field or the ability to demonstrate relevant experience
- Demonstrated experience in a leadership role with experience managing employee’s workloads and coordinating administrative tasks in a timely manner.
- Demonstrated ability to work independently with limited supervision, solve problems and manage and resolve conflict.
