Provide a high-quality Social Work service to patients, clients, consumers and their family/carers to ensure the provision of optimal health outcomes consistent with FWLHD policies, procedures, standards, relevant legislation, and the NSW Health Code of Conduct. This position provides the above Social Work service to the Enhanced Care Team as part of the Integrated Care Team.
- Demonstrate advanced clinical reasoning skills in the provision of clinical care and operate independently with minimum direct clinical supervision to provide a high quality clinical service.
- Participate in planning, implementing, evaluating and reporting on services to promote effectiveness and efficiency of clinical service delivery.
- Provision of psychosocial assessment, counselling, information and advocacy to clients, patients, consumers and their families/carers in a client/consumer centred framework
- Liaise with other care and service providers to ensure seamless continuum of care that meets the needs of clients, patients, consumers and their families/carers.
- Provide appropriate Social Work interventions and support for consumers of the Palliative Care service, in a time of high levels of family stress.
- Identify and act on opportunities for improvement in clinical practice and develop and lead ongoing quality improvement activities in order to improve clinical care, and act on opportunities to participate in clinical research.
- Be accountable for the provision of a high level clinical social work service (including services of a more complex nature) in order to improve the health and wellbeing of people in Far West NSW Local Health District , including exercise of independent professional judgement in solving problems and managing complex situations.

- A bachelor's degree in social work which provides eligibility for membership of the Australian Association of Social Workers. Knowledge, skills and experience to meet the requirements of the NSW Health Professionals Award Level 2.
- Demonstrated ability to work in a health related field and ability to provide a range of Social Work interventions eg psychosocial assessment, case management ,group work, advocacy and education
- Demonstrated ability to work with Bereavement, Grief and Loss in a Palliative Care setting and an understanding of current grief and loss theory.
- Demonstrated ability to work with consumers, families and carers in a time of high levels of family stress.
- Extensive specialist or broad generalist knowledge, clinical proficiency and ability to work with minimal supervision, and collaboratively as a member of a multidisciplinary team.
- Ability to effectively prioritise and organise own work/ caseload.
- Demonstrated commitment to professional development, quality improvement and evidence-based practice.
