About the role
The Hospital Admission Risk Program - Disease Management Team (HARP-DMT) at GV Health are seeking an Exercise Physiologist to join the team.
HARP-DMT is an interdisciplinary team that provide home based services to clients with chronic cardiac and respiratory disease. This is a great opportunity for a physiotherapist who enjoys providing home and community based care to maximise a patient’s independence. The HARP-DMT is a friendly, supportive team that has a well-developed interdisciplinary approach.
You will have:
Broad based clinical experience in the area of assessment and management of people experiencing chronic pain/chronic cardiac and respiratory disease, as well as demonstrated knowledge and understanding of service delivery across the health sector.
This position requires a high level of independent practice as well as the ability to work collaboratively as a team member. Excellent communication and interpersonal skills as well as an understanding of an evidence based approach to person centred care are critical to the role.
This role is open to Grade 1 and 2 applicants. New Graduates will be considered.
