Position Details
Primary Role: REFOCUS Extended Post Care Support - EPCS
Work Location: Nambour, Sunshine Coast Region QLD
Employment Type: Permeant Part Time (30,32 or 34.4)
Salary Range: $35 - $42 per hour
Qualification & Experience: Qualification in Community/Social Services. Must also complete the Hope & Healing Framework Foundational Training 10 modules which will be completed with us as part of your onboarding training.
Reports To: Team Leader
Purpose
- To support young adults to remain living with their carer after they have turned 18 years, if this is their preferred arrangement.
- Administer financial assistance to eligible young people living independently.
- Work with young people around development of a Transition and Financial plan and gaining the skills, knowledge, and networks they need.
- Facilitate opportunities for the young person to participate in peer groups and networking enabling the young person to develop relationships with peers, learn from others, and build a social support network
- Assist Young people to develop and maintain relationships with significant family members, friends, community organisations and government agencies to promote lasting connections and positive outcomes.
- Support and promote engagement with local Aboriginal and Torres Strait Islander Communities and Community Groups
About You
- A team worker – ability to work with various stakeholders, share ideas with a constructive approach to reach common goals focusing on the best interests of the young person.
- Great at relationship building – listening and giving your time. Respecting yourself and others and have professional boundaries.
- Flexibility – be open to new ideas, cope with unexpected change and multitask with ease. Be creative in your thinking and approach each task without judgement and confidence.
- Creative in your thinking and approach life without judgement and confidence.
- You hold a drivers licence (minimum green P’s) as the role will transport young people from time to time.
- Be experienced in person centred case management service delivery and support for young people with complex needs
- Have experience in managing complex caseloads and maintaining a high level of documentation.
- Demonstrated experience and understanding of the factors impacting on young people in the transition from out of home care to independent adult life
- You hold a current Qld Working With Children Check (Blue Card) or are able to obtain one prior to commencement
