Assist with Activities of Daily living, community participation, and travel. Includes but not limited to
- Travelling to Participants’ home and providing assistance and supports with your daily activities (e.g. self-care tasks, feeding)
- Personal Cares; showering, dressing, toileting, hoisting
- Daily Living Tasks; Cleaning, Meal preparation, shopping
- Assisting Participants to attend social outings or activities to increase community participation
- Providing transport for outings, activities, or medical appointments
- Helping Participants work towards and achieve their goals by Developing support plans that are unique to Participants’ individual goals, needs or requests
- Working with allied health workers to develop support plans to help Participants achieve specific goals (e.g. training to catch public transport independently)
- Providing Participants with physical and emotional support
- Working with Participant’s family and other key workers (e.g. school teachers) as needed
- Help Participants to be more independent
- Ensuring that Participants always have choice and control.
Qualification/Requirements
- Certificate III in individual support or relevant industry experience
- NDIS Workers Screening - Yellow Card
- Working with Children Check - Blue Card
- First Aid and CPR Certificate
- Current "C" Class Driver’s License
- Full Comprehensive Car Insurance
- Own Vehicle
