About the organisation

Based in Coburg and working across the Merri-Bek catchment area, you will be working on a dynamic multi-disciplinary HACC PYP Team, assessing clients with a disability, providing clients with services to allow them to remain living independently at home and feeling supported and socially connected. The HACC PYP Assessor is an exciting role undertaking the assessment of people in their homes, under the age of 65, and supporting their access to much needed services.

Duties

HACC PYP Assessors provide home-based face-to-face assessments of clients by completing a comprehensive Living At Home Assessment (LAHA). Working with each client, the Assessor establishes goals specific to the client; develops appropriate strategies and actions, including referral to care services; and develops a Support Plan. The Assessor completes the LAHA and matches and refers clients to appropriate service(s).

The requirements of this role would be to step into the role and hit the ground running, so it is essential to have demonstrated experience working as a HACC PYP Assessor in a previous role and have up to date knowledge of the funding and legislation/program.

This role can start ASAP, and is highly flexible - with both full time and part time hours considered, akong with hybrid work options.

Skills and experience

If you have had prior assessment experience in Community Care, performing home based HACC PYP Assessments within the Aged/Disability Care sector, either in Local government or not for profit organisations, we would love to speak with you about how we can help you to achieve your career or lifestyle goals. You will need a sound understanding of MAC portal use, knowledge of the LAHA assessment tool, HACC PYP/CHSP funding, and hold a tertiary qualification in Allied Health, Nursing or Social Sciences. You will need to be able to to commit to at least three days per week. A police check dated within the last six months is essential.
