Your new role:

• This role is your chance to join the highly skilled HARP Complex Care service at Merri Health to provide coordinated, goal orientated therapy to clients with complex respiratory conditions.
• You will work closely with the respiratory department at RMH to help improve client’s quality of life and reduce avoidable hospital admissions.
• You will be encouraged to share ideas through quality improvement initiatives and supported with ongoing professional development

You will be responsible for:

As a HARP Respiratory Physiotherapist, you will be responsible for:
• Providing the centre based Pulmonary Rehabilitation Program (exercise and education) and home based exercise programs when required
• Opportunity to upskill in HARP Airway Clearance Clinic
• Work closely with clients, their carers, hospital clinicians and GPs to provide integrated client-centred care for clients with respiratory diseases in accordance with best practice guidelines.
• Conducting pre and post assessments (including subjective and objective measures)
• Improve self-management and health outcomes for clients through provision of exercise and education about the optimal management of their condition
• Collaborate with HARP team members to identify and address client’s psycho-social, behavioural, and environmental needs and other needs associated with their complex conditions

What you will need to succeed:

To be successful with Merri you will go above and beyond to achieve great results for our clients, the organisation and yourself. We’re looking for individuals with:
• Tertiary qualification in Physiotherapy and a current registration with the Australian Health Practitioner Regulation Agency (APHRA)
• Experience in providing group exercise programs particularly pulmonary rehabilitation
• Experience in completion of physical assessments, identification of exercise related goals and implementation of evidence-based clinical interventions for clients with respiratory diseases
• Experience in client self-management strategies, motivational interviewing, health coaching, and lifestyle modification – encouraging clients to increase their independence
• Demonstrated ability to work autonomously and collaboratively as part of a multi-disciplinary team.
• Strong organisational, planning, time management and prioritisation skills
